Chez les patients pr\u00e9sentant d\u00e9viation septale post-traumatique et d\u00e9formation externe, l\u2019abord ouvert via une petite incision trans-columellaire permet un contr\u00f4le tridimensionnel des cartilages et des os propres du nez.<\/p>\n<p>Cas clinique type : d\u00e9viation septale majeure secondaire \u00e0 fracture nasale ancienne avec obstruction respiratoire. L\u2019objectif est la septoplastie structur\u00e9e (redressement, greffes de spreader si n\u00e9cessaire) et la correction esth\u00e9tique (dorsum, pointe, base) au cours d\u2019un seul temps op\u00e9ratoire.<\/p>\n<p>Technique : apr\u00e8s dissection, le septum d\u00e9vi\u00e9 est redress\u00e9; les greffes cartilagineuses (septales ou conchales) stabilisent la valve interne, restaurent la sym\u00e9trie et pr\u00e9viennent la r\u00e9cidive. La pointe est raffin\u00e9e par sutures, parfois appuy\u00e9e par greffe columellaire (strut).
